This form will serve as a guide for the cafeteria team to prepare meals. Lunch pickup will take place in the back of the High School Cafeteria complex. Please provide student's first name and last name.

NOTICE:  PLEASE BE INFORMED THAT THE DERRY AREA SCHOOL DISTRICT IS OPEN - WHICH CHANGES OUR SITE TO A CLOSED ENROLLED SITE.  ONLY STUDENTS ENROLLED IN THE DERRY AREA SCHOOL DISTRICT OR PARTICIPATING IN THE E-Academy are eligible for the FREE meals.
Meals may be preordered and picked up at the rear entrance of the HIGH SCHOOL CAFETERIA between 10:15 and 10:45 a.m.  Identification of students will be required.
